Residential Aged & Disability Care Concierge Service
Our Goal
When your care needs become too great and in-home care is no longer viable, residential care is your best option.
Our goal is to assist you in defining your needs and wants, and help you find - and relocate to - the right aged residence for you in the location you desire, within your financial means.
Our Process
We provide a transparent, impartial, professional and compassionate service to our clients.
Having assisted clients for more than three and a half decades, we have created a useful process for decision making. It’s called the "5 Easy Steps 4 Care©". These are the steps we take to help you put in place the right care solutions for you and your family.
For thoroughness, we liaise with your “support team”, be they your family, guardian, power of attorney, financial planner, accountant, lawyer, estate planner, GP or social worker, to ensure all your needs are met.
Our five-step process involves:
-
Establishing your needs: We sit with you to discuss your needs and wants. Members of your support team are included in the discussion. Family members may prefer to have you located in a residential facility closer to their home to allow them greater opportunity to visit. If there are any differences of opinion, we are here to help resolve them.
-
In-Home Assessment: We thoroughly explore whether in-home care is still an option and document why it may no longer be an option.
-
Developing a Life PlanTM: Having established your needs and wants, we will provide you with a comprehensive Life PlanTM. The plan details our recommendations for residential care and support. It may include helpful resources such as independent guides on choosing residential care, how it is financed and your estimated out-of-pocket costs. If a client has an urgent requirement to enter residential aged care, we prepare an Overview (a summary Life PlanTM) about the client which is provided to the facilities being toured to ensure we get a good match.
-
Selecting the appropriate care provider: We will recommend up to four quality residential facilities based on your needs and wants. These recommendations are drawn from the DR Care Solutions bespoke comparative platform on residential facilities. Many of those facilities, and/or their corporate entity, have been personally visited and vetted by Danielle.
Danielle will arrange a personal tour of each facility and will join you, and your family or support team member, on that tour either in person or remotely, depending on travel restrictions.
On making your choice, we will brief the staff within the facility on their role in ensuring your needs and wants are met in accordance with your Overview (summary Life PlanTM).
-
Follow-up and Reviews: Our services does not end with your move to residential care. We remain in contact to monitor that your needs are being met by the facility and your support team are happy with the care arrangements. We have been called on to introduce aged care financial planners to assist clients with strategies to pay for their residential care. In other instances, clients have requested additional companion care and we have arranged in-home carers to provide that additional care to the client at the residential facility.
A Couple Too Late for Home Care
At ages 87 and 83, this couple lived in their own home without support, care or assistance. The husband was living with dementia, the wife suffering cognitive decline, and both were diabetic. They would not allow anyone into their home, they appeared dirty, dishevelled and undernourished. Yet they continued to refuse help from their sons.
'Life Event' Without An Estate Plan
Fit at 78 and living independently, Jana did not expect the severe and debilitating stroke she suddenly suffered. With the resulting permanent disabilities too severe for her to return home, she and her daughter were left in shock, unsure where to begin their search for residential care nor how to even begin organising Jana's estate plan.
Residence Chosen By Grandchildren
Jan was diagnosed with early onset dementia with a rapid cognitive decline. Her husband, Jack, was also struggling with his own serious health conditions; he could no longer keep up with the increased level of care Jan required. For her safety, residential care was the best option; but Jan was concerned her grandchildren would not want to visit her there.
Testimonials

"My sister and I reached out to you in crisis. We were told that Mum could not return home and needed to be urgently placed into care. We phoned you and within a 48-hour period (before a weekend) and during Covid-19 lockdown, you were able to 'pull a rabbit from a hat' and find Mum the perfect place for her end-of-life care.
You moved swiftly and compassionately to ensure Mum could be moved. Unfortunately, Mum's condition deteriorated to a point where the hospital advised she could not be relocated.
We appreciated your help when we most needed it, thank you so much. We are missing Mum every day but getting on with things and hope that now that the lockdown is finished, we can get back to something that resembles normal.
I think there will always be that nagging feeling that something is missing now that Mum is no longer with us. I hope others feel comfort knowing you can help them at such a tumultuous and emotional time, quickly and professionally."

"We recently found ourselves in a position where we had to organise respite care for my mother. She'd been living with us here at home for almost five years. We'd looked after her, everything, every day, and we were really intensely nervous about taking this next step.
We had done initial Googling, we'd hung on the phone for two days trying to get information, and we were more and more confused than ever.
When we spoke with you, you gave us a clarity and a confidence and a surety that put us at ease straightaway. You are just incredible to work with. We were at a massive crisis point, and you just made everything happen and it just took a complete load off of us. It meant we could concentrate on what we need to, witch was Mum, and we got a result and it was incredible how quickly it all happened and you pulled it all together.
We got the best result for Mum and for our family at the same time. So eternally grateful for all your help. Thank you."

"My Mum was based in Melbourne and became very unwell quickly. After being in hospital for a month, we were told she couldn't return home and would need 24/7 medical care moving forward. My brothers and I had 10 days to find her care in Sydney, complicated by COVID-19 restrictions.
In what was an already stressful time - and over the Christmas break - Danielle immediately got to work and was able to shortlist five potential nursing homes in lightning speed time. We walked through all five in the space of a day.
Danielle's wealth of experience within the industry made me feel like I was in safe hands. She knew what to look out for, asked all the right questions, and helped us secure a new home for Mum that, as a family, we (and importantly, Mum), are really happy with.
Navigating the mind field that is aged care homes and aged care living was made infinitely easier with the help of Danielle. I honestly don't think that I would have been able to do it without her!"
